Provider Submission Guidelines
Eligibility
Support is available for families and children with autism and special needs who are currently receiving or seeking services through our partner provider network or community resource groups.
Required Information
Providers must provide complete organization details, client contact information, and specify the assistance type. For balance support, a detailed invoice or ledger of outstanding costs must be included.
Process & Follow-up
Applications are reviewed. We may have to follow up directly with families providers, organizations to confirm details or for more information. The Assistance Award is typically issued directly to the provider on behalf of the family.

Financial Assistance for Balances
The Sade'Marie Foundation provides targeted support for families facing financial barriers to essential care. Our outstanding balance assistance is available for clients with verified special needs who have accrued debt for therapeutic or medical services. Eligibility is determined based on financial hardship, availability and other factors. We work directly with organizations to resolve these balances, ensuring that every child continues to receive the critical advocacy and support they deserve without the burden of unpaid costs.
